Coaching Contract
This Coaching Contract (shall be referred as "Contract" hereinafter) is entered into by and between the parties below, whereby Jennifer Coffey, (shall be referred as "Coach" hereinafter) agrees to provide services, which are described in the related section of this Contract ("Coaching Services").
As a client, I understand that Co-Active® Coaching is a comprehensive process that may involve all areas of my life, including work, finances, health, relationships, education and recreation. I acknowledge that deciding how to handle these issues and implement my choices is exclusively my responsibility. I understand that Co-Active® Coaching does not treat mental disorders as defined by the American Psychiatric Association. I understand that Co-Active® Coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care or substance abuse treatment and I will not use it in place of any form of therapy. I promise that if I am currently in therapy or otherwise under the care of a mental health professional, that I have consulted with this person regarding the advisability of working with a Co-Active® Coach and that this person is aware of my decision to proceed with the Co-Active® Coaching relationship. I understand that feelings are a normal and healthy part of being human and that through a CoActive® Coaching relationship, awareness and curiosity will be brought to my feelings so that I can make more informed choices and move into my desired action. I understand that coaching does not deal with the psychological antecedent to emotions — that is the realm of therapy.
As a client, I am committed to creating a coaching alliance with the Coach that will support me as I clarify and realize my goals and move steadily toward living the life I want to live. The Coach agrees to hold all content of our sessions completely confidential.
Either myself or the coach may terminate this Contract at any time with 48 hours written notice. I agree to compensate the Coach for all coaching services rendered through and including the effective date of termination of the coaching relationship.
Except as expressly provided in this Contract, the Coach makes no guarantees, representations or warranties of any kind or nature, express or implied with respect to the coaching services negotiated, agreed upon and rendered. In no event shall the Coach be liable to me, the client, for any indirect, consequential or special damages. Notwithstanding any damages that I, the client, may incur, the Coach’s entire liability under this Agreement, and my exclusive remedy, shall be limited to the amount actually paid by me, the client to the Coach under this Contract for all coaching services rendered through and including the termination date.
Coach agrees to maintain the ethics and standards of behavior established by the International
Coach Federation “(ICF)” (Coachfederation.org/ethics).
AGREEMENTS/POLICIES/PROCEDURES
COMMITMENT:
You are hiring me as your coach because you want to make significant changes in your life. These changes will sometimes happen fast and sometimes not. This allows the coaching relationship to develop and be powerful, and when the going gets tough we stay in collaboration and work through to the other side. As your coach you have my word that I am 100% committed to you being powerful, successful and having the life you want. If during the coaching relationship there is any reason to change our agreement, my request is that we design a time period during which we will look at the issue/s and create a mutual resolution that honors you, as the client, me as the coach and the coaching relationship we hold together. Upon completion of our time together, if we mutually agree to continue the coaching relationship, we will negotiate new terms and pricing at that time.
PROCEDURE:
Show up on zoom or over the phone, as agreed upon in advance. Please be in a private location where you feel safe and secure to speak openly and honestly. Please have a place to keep notes nearby. I ask that you are completely sober, and stationary for the start of each coaching session. I will not accept coaching sessions while you are intoxicated, imbibing or driving. If the car is your only safe, private option, you must be in a parked position for our coaching session. If you are taking medicine that could alters your consciousness, please email me to discuss.
CONFIDENTIALITY:
Our relationship is completely confidential (with the exclusion of a legal obligation to share information that involves you, or another individual being harmed).
TIME:
We will begin and end our sessions on time, in honor of our commitment. I understand things come up and you may be running late. I will always wait 10 minutes from the start time before logging off and considering it a paid cancellation.
SCHEDULING:
If you need to reschedule I would appreciate at least 24 hours notice. If you have an emergency, we’ll work around it. Missing an appointment without notice is considered a paid forfeited appointment. A rescheduled appointment with less than 24 hour notice is also considered a forfeited paid session.
FEE:
All fees must be paid in advance. As a convenience to both of us, I offer the option of using a credit or debit card for your payments via stripe or paypal. If you would prefer another method of payment (check, bank transfer), please let me know. Fees may increase annually.
IN-BETWEEN SESSIONS:
If you would like to share a success or a problem with me at non-coaching times you may contact me via email, text, or voice message. I promise to respond as quickly as possible, and definitely within two business days.
